start with one and have it adjusted or changed to fit the style and theme
you're looking for.
The benefit to using a template is that using one takes 25% off the per page cost of the design, AS WELL as probably replaces the potential cost of custom graphics. (Depending on what kind of site design you're looking for. If you don't want any changes to the graphics in the template or add any graphics, there's no need for a Custom Graphics charge.)
